  1. Open the form in question
  2. Click Send
  3. On the send screen, there are 3 tabs "Send Via"
  4. The third tab gives you the code to embed < >

Google Slides (on Google Drive) Here is a slideshow I made using Google Slides. What you do is create the show, then click the File menu and "Publish to Web."
